PenCom Enrollment Exercise

Documents required for the Verification and enrollment exercise

To ensure a smooth transition into retirement, it is crucial that you complete both the Data Recapture exercise and the Retiree Enrolment exercise before your retirement date. Please visit any of our offices at your earliest convenience to finalize these important steps.

The original copies of the underlisted 16 documents are to be provided for both exercises:

  1. Letter of First appointment/Attestation
  2. Birth certificate/Declaration of Age
  3. Promotion letter and payslip as at 30th June 2004 (availability to agree with MDA letter)
  4. Promotion letter and payslip between 2004 – 2006, 2007 – 2009, 2010 – 2012, 2013 – 2015, 2016 – 2018, 2019 – 2021, 2022 – 2024 (where applicable indicating grade level and step)
  5. Promotion letter and payslip as at current period (where applicable)
  6. Evidence of transfer of service (where applicable)
  7. Evidence of acceptance of Evidence of transfer of service (where applicable)
  8. Copy of Staff ID card
  9. Evidence of change of name (where applicable)
  10. Record of service
  11. Medical Report (if applicable)
  12. Evidence of extension of service (where applicable)
  13. National Identity Card or Slip issued by the National Identity Management Commission, indicating the National Identity Number (NIN) *346# to verify.
  14. Enrolment registration slip signed by retiree
  15. 1 recent passport (taken against a white background)
  16. Letter of introduction from MDA stating the following and to be signed by two signatories not below(a) Asst. Director Level in HR or personnel dept. &(b) Pension Desk officer of the organization.
  • Date of first appointment 
  • Date of birth 
  • Date of transfer of service (where applicable)
  • Promotion with grade level and step as of June 2004 
  • Promotion with grade level & step between July 2004 to December 2006 
  • Promotion with grade level & step between January 2007 to December 2009 
  • Promotion with grade level & step between January 2010 to December 2012 
  • Promotion with grade level & step between January 2013 to December 2015 
  • Promotion with grade level & step between January 2016 to December 2018
  • Promotion with grade level & step between January 2019 to December 2022
  • Promotion with grade level and step as at current date.
